“How May I Serve You?” Survey Slip (For an additional description of this form, see Part 4 – Share What You Love, of Makeup Optional)

These Survey Slips are an invaluable reference for future follow up. Distribute the slips at each party and request that guests complete and return it to you. Jot notes about items purchased for future follow up with the guest. Another wonderful use of the slip is to ask guests at the party to let you know if they would book a party if the hostess decided to start her own business!
